We accept referrals from hospitals, discharge planners, case managers, and community providers. If you have a patient who is medically stable but needs a safe place to recover, submit the form below and our intake team will respond within 2 business hours.
WHO QUALIFIES
BeAttitude Recuperative Care Home serves adults who are medically stable enough to leave the hospital but still need support before returning to independent living.

Medically stable, no skilled nursing needed

Experiencing homelessness or housing instability

Recovering from illness, injury, or surgery

Able to perform basic self-care with non-clinical assistance

Willing to participate in recovery and transition planning
